PHAR4999 Internship II.

Internship institutions: Independent pharmacies/ Hospital pharmacies

Time of internship: Third-year, summer period (end of the 6th semester).

Internship duration: 30 workdays.

  1. Pharmacies are only considered valid for internship if the residing pharmacist has been practicing for at least five years. Hospital pharmacies for internships must be inpatient treatment institutions, and at least one pharmacist must be on duty in the hospital pharmacy.
  2. No familial relations may exist between the pharmacist and the intern.
  3. The student is expected to contact a pharmacist fulfilling these criteria to negotiate the internship period and details.
  4. Following the agreement of the supervising pharmacist, the internship application form must be filled out and submitted to the internship committee for approval.
  5. If approved, the internship institution is considered finalized, and the student may complete the internship during the declared time period. Application documents must be approved by the internship institution and submitted to the faculty secretariat by April 04, 2025.
  6. Internship application documents and timeline may be accessed at the faculty website.
  7. Both Turkish and international pharmacies/hospitals are eligible to employ interns.
  8. Once the student's internship location is finalized, the Internship Evaluation Form is submitted to the pharmacy/hospital where the internship will take place. (At the beginning of the internship, the student must download it from the website and deliver it to the pharmacist in an envelope.)
  9. At the end of the internship, the supervising pharmacist will complete and sign the form, and send it in a sealed envelope to the department.
  10. Internship committee members will regularly contact internship institutions to ensure that the internship is proceeding according to departmental standards.
  11. Following the internship, the intern will complete and submit an Internship Report according to departmental guidelines.
  12. The Internship Report must be signed and stamped by the supervising pharmacist.
  13. Internship Reports must be submitted to the department by the deadline described in the internship calendar. Students who fail to submit by that date are considered to have failed the class.
  14. Internship documents will be evaluated by an ‘Internship Exam Committee’ formed by at least three members.
  15. Documents will be evaluated and converted to a final grade as follows:
  16. A minimum grade of 70 is required to succeed. This is a pass/fail class; there are no letter grades.
  17. Interns who have delivered a valid report are nevertheless considered to have failed if they fail to attend the internship exam.
