This form has been developed to provide the necessary counseling and support services to our students with disabilities who have been admitted to our University. In order to benefit from our University's educational and social opportunities to the fullest, you first need to complete the form below in full. The information you provide will be kept strictly confidential unless you request otherwise.

In order to benefit from some support services at our University, you may need to submit official documents regarding your diagnosis. For example, visual acuity test results for vision loss, a doctor's report stating your illness for chronic illness, hearing test results for hearing loss, and a DSM-IV diagnostic criteria report for emotional and behavioral disorders, etc.
